Based on LPA Wheatley's observation and record review, the licensee did not comply with the section cited above in that licensee showed a chart with the children's names which she falsified and wrote the month December 2025. This requlation is required and mandatory. This posed a potential health, safety or personal rights risk to persons in care.

Licensee will ensure she is documenting the children (age birth to 2 years) the napping time daily with correct dates and times starting 12/10/25. The records must be kept for 3 years after the child is disenrolled. The licensee will record the napping times every 15 minutes.
