The child walked down the hallway and entered into the classroom where the bathroom is located. The teacher did not have vision of the child once they entered the classroom, which poses an immediate health, safety or personal rights risk to persons in care.

Training description and proof of completion shall be sent to LPA by 07/21/2023.
