Per facility representative, Anahit Zakharyan and licensee, Marine Azizyan that called over the phone, effective tomorrow only 14 children will be present in the home at one time.

During this inspection, Nare Chakhmakchcyan, assistant was present in the home, who stated that she is working at this facility for two weeks and did not have criminal record clearance on file. This poses an immediate health, safety and personal right risk to the children in care.

Per facility representative, Anahit Zakharyan, effective tomorrow 07/01/26, Nare Chakhmachyan will not be present in the facility till grant a criminal record clearance in licensing information system. $500 Civil Penalty was assessed during this inspection. LPA contacted Regional office and verified that Nare Chakhmachyan did not have criminal record clearance on file. On 06/30/26, licensee was not present in the home, who called and informed LPA that she went to Armenia on 06/25/26 due to family emergency and facility will be open during her absence and did not nofity the department. This poses a potential health, safety and personal right risk to the children in care.
