      Based on director statement, they have not completed facility water lead testing yet, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Per director, they will complete required water lead testing as far as they use the facility kitchen water faucet for drinking and making foods by the plan of correction due date and copies of LIC 9276, LIC 9276 and labeled facility sketch will be submitted to LPA Eivazian by 06/30/23.

      Plan of correction: Per director, staff#4 and director will update their pediatric first aid and cpr training by plan of correction due date and copies of certificates will be submitted to LPA Eivazian by 06/30/23.
