Based on record review, the facility did not comply with the section cited above in some parents in classroom #2b and classroom #8 signed children in but not signed the children out which poses/posed a potential health, safety or personal rights risk to persons in care.

Facility director indicated that she will email the parents to remind about sign-in and sign out procedures. Director will discuss with staff members about sign in and sign out procedures. Director indicated that the facility will make sure to check the children sign-in and sign out everyday. Director will forward the email to LPA Nguyen.
