      Administration of Child Day Care Licensing

      Based on observation, interview, and record review. The licensee did not comply with the section cited above. At 1:00 pm, during staff file review and interview, 1 out of 5 staff members did not have their mandated reporter training certificate expired 10/2025. which posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: The director states that they will ask S1 complete updated Mandated Reporter training and submit the certificate to LPA by the due date.
