      Lead Testing

      Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 regulation. The facility owner confirmed that the facility has not completed lead testing for the water outlets as of 08/03/2023, which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility owner states that he will submit a detailed plan to complete the lead testing for the facility by due date to LPA.

      Limitations on Capacity and Ambulatory Status

      Based on observation, interview, record review, the licensee did not comply with the section cited above in 6 out of 6 children in care. LPA Tran observed 6 school-age children were commingling with 7 preschool children and staff members confirmed that the two group age of children were commingling during operation of the summer, which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility owner states he will submit documentation of retraining staff about not to commigle the group ages and will submit a plan to ensure compliancy to condition of their license and waiver.
