      Based on observation and record review, LPA observed 1 expired medication which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility will give back expired medication to parents, replace medication, and submit letter to LPA via email by 5pm on 1/20/26.

      Based on observation, LPA observed the life ring did not have U.S. Coast Guard approved label which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility will puchase a life ring that is labeled as approved by the U.S. Coast Guard and submit photo to LPA via email by 5pm on 1/20/26.

      Lead Testing

      Based on record review, the licensee did not comply with the section cited above in LPA observed that the lead testing was conducted on 10/07/2024 and did not meet the deadline which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: LPA discussed the importance of adhering to the Department's lead testing written directives. LPA provided the ccld.ca.gov website for any licensing updates and infromtation.

      Correction due Oct 23, 2024. Standard 1597.16(a)(1).

      Responsibility for Providing Care and Supervision for Infants

      Based on record review, the licensee did not comply with the section cited above in LPA did not observe an sleep observation log for the infants 18-24 months which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility representative, Veronica Chen agreed to document the sleep observation log for the infants 24 months and younger. Veronica Chen will send a weeks observation log to LPA via email sarah.garcia@dss.ca.gov by 10/30/24 at 5pm.

      Responsibility for Providing Care and Supervision

      Based on LPA's observatio the preschool facility did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA's observed in two seperate classrooms, a single ptreschool child in a restroom with the door closed, not being supervised visually by a staff.

      Plan of correction: The director stated that they will address the Supervision requiements per Title 22 Regulations ensuring that all children must be visually supervised at all times, including when using the restroom or changing. Facility will submit proof to LPA via email that all staff have acknowledged and will maintain compliance of this regulation at all times. Email will be sent by 11/7/22.
