Based on LPA's observatio the preschool facility did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA's observed in two seperate classrooms, a single ptreschool child in a restroom with the door closed, not being supervised visually by a staff.

The director stated that they will address the Supervision requiements per Title 22 Regulations ensuring that all children must be visually supervised at all times, including when using the restroom or changing. Facility will submit proof to LPA via email that all staff have acknowledged and will maintain compliance of this regulation at all times. Email will be sent by 11/7/22.
