Based on observation, the licensee did not comply with the section cited above in the outdoor activity space. There are several fence post bases where the cement has broken off and there is exposed metal. On one metal post base, a cement block is detached. The storage shed has nails protruding from the trim of shed. The shed’s bottom panels on all four sides are jagged, some sides warped, chipped and splitting. Grass is growing in the wood chip area. The exterior wall, facing the shed, has a large piece of stucco missing. This poses a potential health, safety or personal rights risk to persons in care.
