      Personal Rights

      Based on LPA records review and Director Crystal's interview the licensee did not comply with the section cited above, C4 and C6 are missing parent/authorized representative signature and date on LIC613A. Furthermore, C1 is missing parent/authorized representative signature and date on LIC700 which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director Crystal stated she will have parent/authorized representative complete the requested above and submit copies to LPA via email by end of business on 01/16/2026.

      Correction due Jan 16, 2026. Standard 101223(b)(1).
