      Outdoor Activity Space

      Based on observation of the outdoor play yard, LPA observed the black tar like material peeling off and separating from the playground step in several areas. LPA also observed a broken undergound electrical box cover in the grass area in front of the play yard. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility will fix the items listed above and send pictures to LPA showing proof that the issues are resolved. Facility will send pictures to LPA by the plan of correction due date 02/27/2026.

      Fixtures, Furniture, Equipment and Supplies

      Based on inspection of the preschool classroom, LPA observed a piece of laminate flooring trim broken and missing.The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility will fix the broken/missing piece and send LPA pictures to show that the issue is fixed. Facility will send pictures to LPA by the plan of correction due date 02/27/2026.

      101216.3 Teacher-Child Ratio

      Based on observation, at 10:00am there was one teacher and one aide present with 17 children in the preschool classroom. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: While LPA was present, the Director sent an aide to assist in the classroom. The faciltiy will write a plan, which will include specific steps that will be taken, on how the facility will ensure that staffing ratios are met at all times moving forward. The facility will submit the plan in writing to LPA by the plan or correction due date 02/27/2026.
