      Based on record review, the licensee did not comply with the section cited above in three of six staff files reviewed. Where S4, S5 and S6 did not have record of required immunizations available for review during inspection. Which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: DIR will email LPA proof of immunizaitons for S4, S5, and S6. DIR stated they understand that proof of immunizations must be obtained prior to employement.

      Fixtures, Furniture, Equipment and Supplies

      Based on observation and interview, LPA observed a countertop in the 2 yr old classroom with sharp, broken edging and a patio cover with peeling paint, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Licensee will repair counter top edging covering a sink area located next to the entrance in 2 yr old classroom as well as the fiberglass type covers for the pinic benchs in the preschool playgrounds. Director will provide photographic evidence of repairs to LPA via email by 08/29/25.

This requirement is not met as evidenced by:ir as well as the felt-type lining of a wall in the 2yr old classroom, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Licensee will replace toilet seat cover in older preschool bathroom and will remove felt-type lining and make repairs on walls if needed in 2yr old classroom. Director will provide photographic evidence of repairs to LPA via email by 08/29/25

      Plan of correction: Director taped flooring and covered sink during visit, as a temporary fix. SInk does not have water usage, only in the classroom. Director is aware that sink and flooring needs to be repaired.

Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.
