Based on records review and speaking with the Site Supervisor, the licensee did not comply with the section cited above in that S1 was not associated and had not been associated to the facility prior to working at the facility, which poses an immediate health, safety or personal rights risk to persons in care.
A correction plan is recorded. No state verification date is shown.

The Site Supervisor contacted the facility's HR Representative who then associated the employee while LPA was at the facility. Per the Site Supervisor, the HR Representative was aware that S1 was never associated to the facility.
