Based on LPA observation, the licensee did not comply with the section cited above in that Infant # 1 was observed to swaddled with a thick blanket, head on a boppy pillow laying on a mat. This poses an immediate health, safety or personal rights risk to persons in care.

During inspection infant was un-swaddled and removed from boppy pillow. Director will review sleep regulations with infant teachers- meeting minutes and signatures of staff presents should be submitted to LPA by 10/17/2023

Pet Director , she will dissasembled cribs not being used in order to place mats inside the nap room. Pictures of proof of correction will be emailed to LPA by poc due date of 03/11/2022.
