      Administration of Child Day Care Licensing

      Based on observation, interview, and record review, the licensee did not comply with the section cited above in LPA observed that staff 3 (S3) fingerprints were not transferred/associated to the facility which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: During the inspection, the facility representative ensured that S3 fingerprints were transferred/associated to the facility. LPA confirmed on Guardian that the fingerprints for S3 were transferred/associated.

      Personal Rights

      Based on observation, the licensee did not comply with the section cited above in that staff used a harsh tone of voice with a child who was awake. Staff told child to put an item away in cubby- child refused saying they did not want to. Staff proceeded to tell child it’s not what you want and counted to 3 which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Per Director they will conduct a meeting regarding personal rights of a child. Director will submit meeting minutes and staff signatures to LPA by poc due date of of 6/24/25.

      Plan of correction: During inspection Director removed the items making them innacessible to children. Per Director they will meet with staff and submit a written plan on how to maintain in compliance meeting agenda and signing sheet by POC due 8/9/2024.
