During staff interviews, it was disclosed that unqualified aide (S2) has been transporting children without supervision of a teacher since Janurary 2026 which poses a potential health, safety and personal rights risk to children in care.

Based on interview with third party, it was disclosed that the facility was late picking up children from a school several times during the period of August 2025 through Decemeber 2025, which poses a potential health, safety and personal rights risk to children in care.

The Vice President of Operations states that she will conduct a training with all drivers regarding the procedure if drivers willl be late to pick up children for afternoon care. The Vice President of Operaitons will have all attending staff sign the meeting agenda. Vice President of Operations will send proof of the signed agenda to the Department via by 5/04/2026.
Based on review of documentation and interviews, Staff #1 tapped on the breaks while children were in the school van, to show what can happen if they were not seated properly resulting in an injury to Child #1. This is a form of intimidation/threat and is a immediate risk to the heath and safety of children in care.

Director states that the facility immediately terminated Staff #1 upon learning of the injury. There will be a meeting with all drivers for the facility sites to address proper safety procedures for driving. At the current site, only she, Teacher Candis Cammon and Yicela Mercado will be driving the school age children to and from school and they have all been reminded of proper procedures to take to protect the health and safety of children during transport.
