Based on the record review and interview, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file review the LPA was unable to review sleep logs for C1 and C2.

Licensee understands all children under the age of two must have a 15 minute sleep log on file and available for review. If the child sleeps multiple times a day, the sleep log must be filled out multiple times a day. Licensee agrees to submit a written statement of understanding and submit to the Department on or by POC due date of 7-27-2026.
