SeriousBased on observation, the licensee did not comply with the section cited above in that the door exiting to the parking lot from the Owls classroom and the door exiting to the outside storage closet were observed to be accessible by children. LPAs observed the 2 doors (in the Butterfly classroom) leading to the outside to be accessible by children which poses/posed a potential health, safety or personal rights risk to persons in care.
A correction plan is recorded. No state verification date is shown.

Director will install hook latches to the top of the 2 doors in the Owls classroom and the 2 doors in the Butterfly classroom. Director will submit photos of the corrections via email by POC due date.
