Based on observation and interview, the licensee did not comply with the section cited above by not ensuring the outdoor play area is free of sharp edges and holes in the border of the outdoor play area which posed a potential health, safety or personal rights risk to persons in care.

Site Director stated that she will talk to the church to submit a request to repair the border in the outdoor play area. Site Director stated that she will duct tape the areas temporarily while a permanent solution is in place. Site Director stated she will submit a written plan for the repairs no later than 10/02/2026.

Based on interviews and record review, the Facility did not ensure that Staff #1 has a criminal record clearance prior to working at this facility, which poses an immediate Health and Safety risk to persons in care.
A correction plan is recorded. No state verification date is shown.

Director has submitted LIC9163 for staff #1 on 6/20/22. LPA verified that staff #1 has received clearance as of 6/18/22. Director stated she will review the CCL video on Background Check Requirements for Caregivers. Director states she will submit a summary of the video as well as a written plan on how to ensure that all staff members receive criminal record clearance prior to employment by 8/29/22.
