Center staff self-reported to Licensing that on August 18, 2022, a lack of supervision lead to a child being left unattended in the classroom. This poses and immediate risk to the health and safety of children in care.

Regional Director Nicole Rowe and Cesar Chavez Center Manager Andrea Nolasco had a meeting with T1 on 8/23/22. The meeting and incident were documented and required T1'’s signed acknowledgment of her failure to uphold Catalyst performance/supervision standards.
