     Administration of Child Day Care Licensing

     Based on record review, the licensee did not comply with the section cited above in three counts of Staff S2, S3, and S4 does not have current Mandated Reporter Certification which poses a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Site Supervisor shall provide LPA Barnes current Mandated Reporter Certifications for Staffs S1,S2,S3 by due date via email. Also, Site Supervisor will also provide a POC to ensure all required employee documents are kept current.

     Correction due Sep 17, 2025. Standard 1596.8662(b)(1).

   • MinorCalifornia: Type B

     Personnel Records

     Based on record review, the licensee did not comply with the section cited above in one count Staff S3 did not have TB clearance which poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Site Supervisor shall provide LPA Barnes current Immunizations-TB for Staff 3 by due date via email. Site Supervisor will also provide a POC to ensure all required employee documents are kept current.
