Based on the observation the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the facility tour LPA observed C4 being placed in the playpen upon arrival. As the tour continued LPA observed C4 awake with a blanket over him/her.

Licensee agrees to read the regulations and write a statement of understanding to the Department on or by POC date. Licensee understands loose articles and objects cannot be placed in the play yards. Safe Sleep regulations were provided.

Based on the record review the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review the LPA was unable to review an LIC 9224 for C4.

Based on the file review the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review LPA was unable to review MMR and TB records for S2. Licensee stated there is no records available for review for S2.
