Based on observation and record review, the licensee did not comply with the section cited above in that from 10:23am - 10:27am there was 1 Teacher's Aide supervising 8 children without being directly supervised by a Teacher. In this instance there were 0 teachers visually observing and supervising 8 children. Upon notification, the Director immediately rearranged staff to comply with this requirement. This poses an immediate health, safety or personal rights risk to persons in care.

Facility agrees to submit a detailed list on which staff are considered fully qualified teachers, teachers, teacher's aides with units, and teacher ‘s aides without units. This list will include how each staff can be considered when it comes to teacher to child ratio. This list will also include whether staff can escort children to the restroom. All teachers will sign and date this list to acknowledge receipt of this information. This will be submitted to the Department by the POC due date.
