Based on record review and interview, there were at least 3 incidents where a child in care sustained a minor injury and no facility staff visually observed the child during the incident. This poses/posed an immediate health, safety, or personal rights risk to person in care.

Facility agrees to conduct training with all staff regarding visually supervising all children. The facility will submit a copy of a sign in sheet for the training, training agenda, and training materials to the Department by the POC due date.

Based on observation, the licensee did not comply with the section cited above, LPA observed the carpeted areas and rugs in the classrooms and throughout the facility to having staining of grim and dirt which poses a potential health, safety or personal rights risk to persons in care.
