Director made the affected water outlet inaccessible by covering with duct tape and ceased operation of the water outlet during the inspection. Director stated she will contact a plumber to replace the fixture within two weeks. Director stated she will request a waiver for alternative portable water. Director agrees to update Plan of Operation to include corrections made for addressing the lead exceedance. Director stated she will conduct a follow up water sample as outlined in Written Directives section 101705 and submit all required documentation to CCL by 6/24/22.

Based on interview, it was determined facility did not ensure children were accorded dignity in his/her personal relationships with staff by Teacher #2 yelling at children on at least one occasion. This is a potential risk to the health and safety of children in care.
