      Buildings and Grounds

      Based on observation, the licensee did not comply with the section cited above as one bottle of hairspray and some bottles/containers of cleaning products were stored in a manner that made them accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director will create a written statement that all items that could pose a danger to children, including cleaning products, activity products, or anything else with a "keep away or out of reach to children" should be stored in a manner that makes them inaccessible to children in care at all times. Director will review and have all staff sign this written statement and email it to LPA Uribe at christina.uribe@dss.ca.gov no later than the due date of 12/13/2025.

      Health-Related Services

      Based on record review, the licensee did not comply with the section cited above as some children's Parent Consent for Administration of Medication (LIC 9221) forms were either missing or incomplete which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Director will have all needed forms completed by the child's parent/guardian. Director will ensure that these forms are completed correctly. Director will email a copy of these signed and completed documents to LPA Uribe at christina.uribe@dss.ca.gov no later than the due date of 12/13/2024.
