MONTEREY PARK, CA 91754 FACILITY NUMBER: 191610105 VISIT DATE: 08/07/2026 Facility on was cited for no certificate of proof for Licensee Blanchard dated 9/29/25; Licensee did not have certificate to show LPA Calderon. LPA Calderon gave a deficiency for Mandated Reporter certificate not in Licensee file or at the facility for review. Civil Penalty was given for repeat violation that is maintained uncleared.
In addition, during today's visit LPA Calderon observed door where Jacuzzi is located in the Jacuzzi Room, door was observed locked (door knob faces the living room where children are cared for) and LPA observed the door to have a high to reach inaccessible latch at the top of the door and a crib in front of the door for inaccessibility; consultation was given to ensure jacuzzi is maintained locked and inaccessible. Jacuzzi has no water and observed with a cover; per Licensee E.Blanchard jacuzzi is not in use and will find someone to lock the jacuzzi itself for additional inaccessibility.
Per California Code of Regulation Title 22 (1) deficiency given with a civil penalty of a repeat violation within 12 months. Appeal Rights were provided and an exit interview was conducted with Licensee ERNESTINE BLANCHARD. DATE: 08/07/2026 DATE: 08/07/2026 Page: 4 of 4
