Based on interview and record review, the licensee did not comply with the section cited above in that, C1 and C2 did not have their immunization records in file, which poses a potential health, safety or personal rights risk to persons in care.

Based on facility Lead sampling tests it was discovered that Outlets F, G, and H had high level of lead. This poses a potential risk to the health of children in care.

Director posted the lead sampling test results. Director placed signs on the outlets to make sure that staff and children do not use the faucets. Director is utilizing filtered water dispensers as another source of water and using personal water bottles as source of drinking water. Director stated they will replace the faucets and are scheduled to retest on 12/13/22. Director will submit retest results by 12/31/22.
