     Based on observation, the licensee did not comply with the section cited above during the facility tour of the toddler classroom LPA observed 7 toddlers and one staff (S2) in classroom number 1 which posesa potential health, safety or personal rights risk to persons in care.

     Plan of correction: Owner and LPA discussed the ratio for toddler component during the time of the visit. Owner will submit a plan of action to the department by POC due date 03/11/2026.

     Responsibility for Providing Care and Supervision for Infants

     Based on interview and record review, the licensee did not comply with the section cited above staff stated that they do not complete 15 minute physical sleep check which poses a potential health, safety or personal rights risk to persons in care.

     Plan of correction: LPA and owner discussed the infant sleep regulations and a copy of the regulation was left at the facility during today's inspection. Owner will staff complete the 15 minute sleep check and send documentation to the department for two weeks by POC due date 03/11/2026.

     Based on record review, two staff files reviewed had expired Mandated Reporter training for child care providers, which poses a potential health, safety or personal rights risk to persons in care.

     Plan of correction: The director stated she will have staff complete the mandated reporter training for child care providers and submit proof of completion to licensing by the due date.
