Based on record review, the licensee did not comply with the section cited above. In reviewing staff files and personnel associations, it was determined that staff#3 had been associated to the main facility (FSA Moreno Valley) and not the facility currently employed in. This poses/posed a potential health, safety or personal rights risk to persons in care.

During the inspection, FAS Human Resources staff, was able to transfer the criminal record clearance and associate staff#3 to the facility's license. Site Supervisor agrees to submit a written plan as to how they will assure compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 3/31/2026. *Please note that this is a repeat violation within 12 months and a $250.00 civil penalty is being assessed*

A toddler was able to leave the classroom due to the gate being broken an in disrepair while staff was changing another child's diaper. Facility director was able to see the toddler walking away from the classroom to the common area through the surveillance cameras. Child was taken back to the classroom This is an immediate risk to Health, Safety & Personal Rights to the children in care.
