Based on observation, the licensee did not comply with the section cited above when the outdoor play equipment had sharp edges and was not in good condition, which poses/posed a potential health, safety or personal rights risk to persons in care.
Plan of correction: Per facility representative, the plastic truck, the kitchen and the table will be removed from the outdoor activity space. Facility will assign a staff member to conduct weekly safety inspections of the outdoor space.

Indoor Activity Space
Based on observation, the licensee did not comply with the section cited above when the floor under and behind one toilet in the children bathroom was brown in color and the floor was peeling. This poses/posed a potential health, safety or personal rights risk to persons in care.
Plan of correction: Per Facility representative, we will seek advise on spot removal and either utilize tile paint or remove and replace tiles. A picture will be submitted to the Department once completed.

Based on observation, the licensee did not comply with the section cited above when peeling paint was present on the walls of the classrooms and in the outdoor activity space. In addition four electrical outlets were missing child safety plug covers. This poses/posed a potential health, safety or personal rights risk to persons in care.
Plan of correction: Per facility representative, contact paper will be placed over the areas by 4/12/23, until a permanent solution can be remedied. Child safety covers were installed today. We will have a teacher meeting and the teachers will be required to conduct morning safety checks. Teachers will document the safety checks. Documentation of the checks and pictures will be submitted to the Department.
