Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2 and C8 were missing proof of the written medical assessment which poses a potential health, safety or personal rights risk to persons in care.

Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 04/02/2026.
Based on record review, the facility did comply w/ith written directives outlined in PIN 21-21-CCP and failed to have the 334844229-FLORIDA STREET DISCOVERY CENTER, water sources tested for safe lead levels of 5.5 ppb or below, which is a potential risk to the health and safety of daycare children.
