Based on record review, the licensee did not comply with the section cited above, LPA observed 2 out of 3 child files did not have vaccination records on file. One infant has been in care for 6 months and no immunization record has been obtained from parent. The other infant started care two weeks ago and parent has not provided immunization records which poses a potential health, safety or personal rights risk to persons in care.
A correction plan is recorded. No state verification date is shown.

The licensee sent one infant home and has not returned to care since 04/21/2026. The licensee was able to provide the other infants immunizations record on 04/20/26. The licensees reported they will write a statement of understanding and will submit to LPA by POC due date.
