      Responsibility for Providing Care and Supervision

      Based on observation and interview, the licensee did not comply with the section cited above. While touring the facility, LPA observed Staff 2 escorting preschool children to the restrooms.The boys went into the boy’s restroom and the girls went into the girl’s restroom. Both restrooms are next to each other. Staff 2 came out of the boy’s restroom and girls were in the restrooms with no staff supervision, which poses a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Designated Representative stayed with the girls and provided supervision to assist Staff 2. LPA provided a copy of the regulation. Director stated a staff training will be conducted on care and supervision to all staff. Director will submit to the Fresno South Licensing Office a training outline and a sign in sheet by 09/30/2025.

      Based onrecord review, the licensee did not comply with the section cited above inDuring staff file review LPA observed that 5 out of 5 staff did not have a current mandated reporter training certificate in file; which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Facility represenative stated that staff will complete mandated reporter training by above poc date and provide copies to LPA.
