This requirement is not met as evidenced by... Based on interview and record review, Area Manager informed LPAs that on 12/29/25 an inappropriate incident happened between two children.
A correction plan is recorded. No state verification date is shown.
Recorded plan of correction
Area Manager acknowledged th lack of proper care and supervison for the two children occured. Area Manager informed LPAs that all staff are required to retake Health and Safety courses and additional in-person protocol trainings. A written plan of correction also needs to be submitted by 1/8/26 before the close of business day (5:00 PM). This written plan of correction will detail how such occurences will be avoided moving forward. Pictures of returned LIC 9224's should also be sent to LPAs before the end of the week (1/9/26).
