      Criminal Record Clearance

      Based on observation and record review the licensee did not comply with the section cited above in one staff out of five were not finger print cleared and associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.

      Plan of correction: Owner stated that staff left the facility immediately. Owner stated that staff will refinger print and will not be allowed at the facility until they are cleared and associated to the facility.

      Based on observation and record review, the licensee did not comply with the section cited above in 1 out of 5 staff did not have a completed mandated report certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

      Plan of correction: Owner stated that staff will complete the mandated reporter training and owner will send a certificate of completion to the LPA by the above POC date via email or text message.
