Based on interviews and record review Staff #1 falsified the subsidy program attendance documentation , claiming C1 and C2 were in attendance on 05/26/2026 when they were not. This poses a potential health and safety risk to the children in care.

Licensee stated she is no longer is filling out the paperwork, she is having the parents fill it out themselves. She stated she did it to help the parent out since they told her she was at risk of losing the program but understands what she did was wrong. Licensee submitted a Declaration form to LPA.
