Based on observation, interview, at 8:51am S1 restrained C1 on the highchair while transitioned C1 from preschool room to toddler room. the licensee did not comply with the section cited above in [count] out of which poses an immediate health, safety or personal rights risk to persons in care.

S1 removed C1 from the highchair immediately. Director stated they will provide personal rights training to all staff and provide an attendence sheet. Director will speak with S1 regarding the incident and provide additional training. Director will submit written POC to licensing office by the due date.
