     Administration of Child Day Care Licensing

     Based on record review, the licensee did not comply with the section cited above. 3 out of 3 staff did not have valid mandated reporter certificate. This poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director stated staff will complete correct mandated reporter training and will send certifcates to LPA.

     Correction due Oct 1, 2024. Standard 1596.8662(b)(1).

   • MinorCalifornia: Type B

     Based on observation, interview and file review, the licensee did not comply with the section cited above. The facility is licensed for room 9 only, based on today's visit, preschool is conducting in room 51. During interview with Director, Director stated the preschool moved to room 51 back in 2020 and the play yard is located in the same area but has been remodeled. This poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director stated she will notify her supervisior to send an updated LIC 999 (sketch) to LPA by POC due date.

     Lead Testing

     Based on record review, the licensee did not comply with the section cited above. LPA observed a functioning drinking fountain in the classroom and accessible to children in care. Lead testing for water fountain could not be provided to LPA. This poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director stated they will conduct a lead testing for water fountain and will provide a copy to LPA once completed. In the meantime, they will place over the water fountain to ensure it is inaccesible to children.

     Correction due Oct 3, 2024. Standard 1597.16(a)(1).

     Personnel Requirements

     Based on record review, the licensee did not comply with the section cited above. Health screening for staff #2 and staff #3 could not be provided and was not observed in their file. This poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director stated she wll send a copy of staff #2 and staff #3's health screening report.

     Correction due Sep 20, 2024. Standard 101216(g)(1).

     Child's Medical Assessments

     Based on record review, the licensee did not comply with the section cited above. Child #1 and Child #5 did not have a physican report on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director stated she will send a copy of the physican report for child #1 and child #5 to LPA.

     Correction due Sep 20, 2024. Standard 101220(a).

     Immunizations

     Based on record review, the licensee did not comply with the section cited above. Child #5 did not have a complete immunization record on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

     Plan of correction: Director stated she will send a copy of imunization records for child #5 to LPA.
