      Based on observation, and interview, 12:09pm a preschool age child walked into the classroom alone and Lead teacher stated child was in the bathroom located outside the classroom. The licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.

      Plan of correction: LPAs requested for the director to submit a written plan of correction on making sure that staff are supervising children at all times. Submit written plan by the due date to LPA at carmen.odom@dss.ca.gov

      Plan of correction: Staff stated they will inform the maintenance department to fix the carbon monoxide detector and notify the licensing department when the carbon monoxide has been fixed.
