Based on observation and record review, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. LPA observed staff (S1) present and assisting with care and supervision at the facility. S1 is fingerprinted, but is not is not associated to the facility. S1 stated she has been working at the facility since March 2022
Recorded plan of correction
Director emailed transfer clearance with LIC 508 and drivers license to duty officer today (7/7/22). Director also agreed that S1 will not work until S1 is associated to the facility. Director will submit a written plan on how to prevent a similar occurrence in the future. Written plan must be submitted by end of day 7/22/22
